Transcranial magnetic stimulation(TMS)
는 머리를 절개하지 않고 국소 대뇌피질을 인위적으로 자극할 수 있는 기술이다. 머리에 배치한 코일을 경유하여 강한 자기장의 변화를 대뇌 피질내에 발생시킨다. 그러면 뇌조직에는 유도전류가 발생하며 이를 이용하여 신경세포를 작동시키는 것이다. 보통은 별 문제가 없지 가끔 고통이 수반되기도 한다. 원래 TMS는 뇌의 운동성 영역을 자극하고 근육의 반응을 관찰하는 방식으로 뇌 척수 신경계를 조사할 목적으로 개발되었다. 보통의 TMS는 일련의 자극을 연속하여 줄 수는 없고 한번의 펄스를 주는 장치이지만, 반복적으로 자극을 가할 수 있는 rTMS는 보다 지속적인 대뇌피질의 작용을 유지 할 수 있다. 요즈음 신경과학자 들에게 부쩍 관심의 대상이 되고 있다.
rTMS
는 정상인의 motor speech area를 자극하여 자극이 주어지는 동안 실어증을 유발하는 실험으로 잘 알려져 있다[참고문헌1,2,3]. 또한 왼쪽, 오른쪽 전두엽을 자극함에 따라서 기분의 변화가 관찰되기도 하였다[참고문헌 4, 5, 6, 7]. 또한, 운동 학습[참고문헌 8], working memory[참고문헌 9], 시각인지와 visual attention에 대한 것도 연구되었다[참고문헌 10]. 한편 강력한 자극에 의한 간질 증상을 보이는 위험성도 보고되었다[참고문헌 11,12,13]. TMS에 의한 효과는 근육의 반응을 정량적 분석을 통해서 매우 정밀한 수준으로 측정할 수 있다. 이와 같은 방법을 이용하여, TMS의 개척자들은 운동피질의 자극은 corticospinal 뉴런을 직접적으로도 활성화 시키고 '시냅스를 경유(transsynaptic)'하여서도 활성화 시킴을 밝혔다. 직접저이냐 간접적이냐는 TMS의 강도와 자극코일의 방향에 의존한다.
최근에
Tomas Paus 와 그의 동료들은 frontal eye field를 rTMS 자극하고 두정엽의 안구운동 영역의 transsynaptic activation 을 설명하기 위하여 PET를 사용하였다[참고문헌 14]. 오랫동안 기다려졌던 이 기술적 발전은 앞으로 많은 기여를 할 것으로 기대된다. 저자들이 이야기 하기로는 사람 뇌의 구조적 연결을 밝힘에 있어 새로운 기술을 소개한다고 밝히고 있다. TMS와 PET를 동시에 사용하는 이 기술을 이용하여 정상인과 환자의 비교를 통하여 뇌의 구조적 차이를 조사 할 수 도 있다. 이 분야에서 또 다른 중요한 목표는 TMS와 functional - MRI와의 결합이다. Functional- MRI는 PET 보다 시간분해능, 공간분해능 측면에서 다소 뛰어나다. TMS와 동시에 사용된다면 뇌의 작동을 좀더 잘 이해할 수 있을 것이고,기능적 신경의 구조를 더 세밀히 밝힐 수 있을 것이다.
